The Coordinating Center provides the coordination of programs and services that people with complex health and disability needs must have to thrive at home and in the community. With 30 years' experience The Center is recognized for innovation in community living, life care planning, housing and community developmen, health and aging, as well as consulting services touching the spectrum of disabilites and complex health and social issues. Operating statewide, this mission directed non profit organization provides much of its core services under the auspices of Medicaid funded home and community based services. The Center is widely known for addressing the needs of people in nursing homes who wish to transition from the facility to homes of their own, or who reside in the community and wish to avert nursing home placement. The organization is certified unde the Maryland Non Profits' Standards for Excellence. Its care management (coordination) program is accredited by URAC.
